• Adults under 5 ft: Subtract 2.3 kg per inch below 60
• Pediatric (Traub): IBW (kg) = (Height in cm² × 1.65) / 1000
• Tidal Volume: TV (mL) = IBW (kg) × selected TV (mL/kg)
